There are many forms of action research. The action research we use in a PLC or study Group is called Collaborative Action Research and involves a team of practitioners working together on an issue.
CAR is a process in which participants systematically examine their own educational practice using the techniques of research, for the purpose of increasing learning of students, their teachers, and other interested parties.

Read an early version of Sagor's text here
Richard Sagor describes Collaborative Action Research as conducted by teams of practitioners, enabling teachers: (1) to improve student learning, (2) to improve their own practice, (3) to contribute to the development of their own profession, and (4) to overcome the isolation commonly experienced by classroom teachers.
We recommend for Action Research the Ferrance Model. It includes two points for gathering data. It ensures that all actions are data-driven.
Follow these steps
Identification of problem behaviour/Issue
Something planned by the schools/ an issue raised by the Division/ Refined for clarity/ Branstorm
Gather Data on issue
Focus groups/ surveys/observations/ journals can give you a snapshot. Make it uncomplicated.
Intepret Data and Decide on action
What is the nature of the issue? What does the data say? What does theory say?
Develop a systematic intervention with a theory of action and implementation plan
Implement and Evaluate
